About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-027 - AMV Mishap Claims One Soldier's Life

A Private First Class assigned to 1st Cavalry Division, Fort Hood, Texas, died in an Army motor vehicle mishap 14 January 2019 at the National Training Center, Fort Irwin, California, at 1900 local. The Soldier was operating a Bradley Fighting Vehicle during a movement with limited visibility when he drove off the side of a 17-foot wadi and came to rest inverted.

ENGAGEMENT TIPS:

While the investigation into this mishap is ongoing and we do not know the exact details yet, here are some actions you can take to mitigate a similar incident from happening in your formation:

  • When possible, conduct a thorough METT-TC analysis, including a real-time risk assessment, before tactical movements.

  • Ensure your crews are properly trained and demonstrate their ability to use night vision systems.

  • Observe your crews rehearsing emergency and rollover egress drills.
