About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

PLR 20-044 - Water-Related Mishap Claims One Soldier's Life

A Specialist assigned to Fort Riley, Kansas, died in an off-duty water-related mishap 2 May 2020 in Randolph, Kansas, at 1400 local. The Soldier was riding in his personal kayak when another Soldier noticed he was no longer in the cockpit. Multiple law enforcement and search-and-rescue agencies were involved in the search, later recovering his body.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the second fatal off-duty water-related mishap of FY20 and below the number of similar fatalities from this time last year.

 

 

PLR 20-028 - Scuba Diving Mishap Claims One Soldier's Life

A Staff Sergeant assigned to Okinawa, Japan, died in a scuba diving mishap 2 February 2020 at 0959 local. The Soldier was reportedly handing the ship’s captain his oxygen tanks when he began yelling and submerged below the water. The Japanese coast guard was notified to conduct the recovery and located the Soldier’s body the following day.

Since FY16, the Army has lost an average of 15 Soldiers a year to off-duty Personal Injury Other (PIO) mishaps. This was the first fatal PIO water-related mishap in FY20 compared to two PIO water-related fatalities this time last year. If history teaches any lessons, expect the number of PIO water mishaps to rise with the spring and summer temperatures. Now is the time for leaders to emphasize safety during off-duty water activities.

 

 

PLR 19-064 - Swimming Mishap Claims One Soldier's Life

A Specialist assigned to Fort Sam Houston, Texas, died in a swimming mishap 14 September 2019 in Spice Wood, Texas, at 1300 local. The Soldier was cliff diving with others from his unit when he slipped, fell into the water and failed to resurface. His body was later recovered by authorities.

PLR 19-060 - Water-related Mishap Claims One Soldier's Life

A 35-year-old Soldier assigned to the Alaska Army National Guard died in a water-related mishap 8 June 2019 near Fort Greely, Alaska, at 0230 local. The Soldier was dipnetting in the Copper River when he was reportedly swept away by the current.

PLR 19-058 - Swimming Mishap Claims One Soldier's Life

A 20-year-old Soldier stationed at Fort Carson, Colorado, died in a swimming mishap 8 June 2019 at Paradise Cove near Guffey, Colorado, at 1516 local. The Soldier was swimming with other Soldiers when he submerged. Attempts to revive him were unsuccessful.
