About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-077 - Tree Trimming Mishap Claims One Soldier's Life

A 59-year-old AGR Soldier assigned to the West Virginia Army National Guard was killed in a tree trimming mishap on 23 June 2018, in Bridgeport, WV. The Sergeant First Class was operating a chainsaw while in a tree when he was struck by a limb. He was wearing proper personal protective equipment.

PLR 18-076 - Training Mishap Claims One Soldier’s Life

A 25-year-old Specialist assigned to U.S. Army Reserve Command died in association with a training event conducted on 20 June 2018 on Camp Blanding, FL. The Soldier was attending the Basic Leader Course and participating in a Land Navigation Course Evaluation when he failed to report to the accountability formation at the conclusion of the evaluation. His body was found two days later following an extensive search. The USACRC is leading a centralized accident investigation into the mishap.

 

 

PLR 18-041 - Maintenance Mishap Claims One Soldier's Life

A Specialist assigned to U.S. Army Special Operations Command, Joint Base Lewis-McChord, Washington, died in a maintenance mishap in the unit motor pool 7 February 2018 at 0645 local. The Soldier was conducting maintenance on an M984 HEMMT wrecker when he reportedly fell from the vehicle while it was in motion and was struck by the rear tires.

PLR 18-020 - Training Exercise Mishap Claims One Soldier's Life

A Specialist assigned to 3rd Infantry Division, Fort Stewart, Georgia, died 12 December 2017 following a training exercise mishap on the installation. The Soldier was participating in a live-fire exercise when he was struck in the head and left bicep by fired 5.56 rounds. He underwent surgery but died from his injuries six days later.

PLR 18-016 - Fall Mishap Claims One Soldier's Life

A Sergeant First Class assigned to the 4th Combat Aviation Brigade, Fort Carson, Colorado, died in a fall mishap 1 December 2017 at 0850 local in Colorado Springs, Colorado. The Soldier was discovered dead at his off-post residence. Initial findings indicate the Soldier died from a suspected head injury after falling in his bathroom.
