About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-067 - ACV Mishap Claims One Soldier's Life

A 22-year-old Private First Class assigned to Fort Hood, Texas, died in an Army combat vehicle mishap 5 September 2019 on the installation at 1640 local. The Soldier was performing repairs on an M2A3 Bradley Fighting Vehicle when the main engine access panel fell and struck him on the head, resulting in a fatal injury.

 

 

PLR 19-066 - Aviation Mishap Claims One Soldier's Life

A 33-year-old Major assigned to Fort Polk, Louisiana, died in an aviation mishap 26 September 2019 on the installation at 0034 local. The crew was conducting a MEDEVAC mission when the aircraft crashed during landing. The USACRC is leading a safety investigation into the mishap.

 

 

PLR 19-065 - PMV-4 Mishap Claims One Soldier's Life

A 26-year-old Specialist assigned to Fort Stewart, Georgia, died in a PMV-4 mishap 20 September 2019 in Hinesville, Georgia, at 0550 local. The Soldier was traveling on a highway when he was ejected from the vehicle, which landed on him. He was pronounced dead at the scene. Speed and seat belt use are unknown at this time.

PLR 19-064 - Swimming Mishap Claims One Soldier's Life

A Specialist assigned to Fort Sam Houston, Texas, died in a swimming mishap 14 September 2019 in Spice Wood, Texas, at 1300 local. The Soldier was cliff diving with others from his unit when he slipped, fell into the water and failed to resurface. His body was later recovered by authorities.

PLR 19-063 - Training Mishap Claims One Soldier's Life

A Private First Class assigned to Fort Sill, Oklahoma, died following a training mishap 26 August 2019 in Oklahoma City, Oklahoma, at 1634 local. The Soldier was digging a hasty fighting position when he began showing signs of dizziness. He was treated with ice sheets at the scene before being transported via ground ambulance to a community hospital. The Soldier was later air evacuated to an Oklahoma City hospital and placed in the intensive care unit, where he died.
