Aviation Mishaps

Immediate telephonic notification required for Class A, B, and C mishaps:

1. Notify the U. S. Army Combat Readiness Center (24 hours a day, 7 days a week) immediately by phone at (334) 255-2660 (DSN 558) or (334) 255-3410 (DSN 558)
2. Enter an Initial Notification into the ASMIS 2.0 Mishap and Near-Miss Reporting Tool.
3. Worksheet may be attached to an email and sent to usarmy.rucker.hqda-secarmy.list.safe-operations-offi@army.mil

Ground Mishaps

Immediate telephonic notification required for Class A and B mishaps both On- and Off-duty:

1. Notify the U. S. Army Combat Readiness Center (24 hours a day, 7 days a week) immediately by phone at (334) 255-2660 (DSN 558) or (334) 255-3410 (DSN 558)
2. Enter an Initial Notification into the ASMIS 2.0 Mishap and Near-Miss Reporting Tool.
