Immediate Notification Instructions

This section provides specific instructions for making immediate notification to the Army Combat Readiness Center of a Class A, B, or C Aviation accident or a Class A or B Ground accident.

Aviation Mishap

Immediate telephonic notification required for Class A, B, and C mishaps.

1. Notify the U. S. Army Combat Readiness Center (24 hours a day, 7 days a week) immediately by phone at DSN 558-2660/3410 or commercial (334) 255-2660/3410
2. Enter an Initial Notification into the ASMIS 2.0 Mishap and Near-Miss Reporting tool.
3. Worksheet may be attached to an email and sent to usarmy.novosel.hqda-secarmy.list.safe-operations-offi@army.mil

Ground Mishap

Immediate telephonic notification required for Class A and B mishaps both on and off duty.

1. Notify the U. S. Army Combat Readiness Center (24 hours a day, 7 days a week) immediately by phone at DSN 558-2660/3410 or commercial (334) 255-2660/3410
2. Enter an Initial Notification into the ASMIS 2.0 Mishap and Near-Miss Reporting tool.
