About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-039 - Pedestrian Mishap Claims One Soldier's Life

A First Sergeant assigned to 2nd Armored Brigade Combat Team, 1st Infantry Division, and attached to U.S. Army Europe, died in a pedestrian mishap 5 February 2018 in Parsberg, Germany, at 0130 local. The Soldier was struck by a train while walking near the station.

  • All train tracks are private property. Never walk on tracks; it's illegal trespass and highly dangerous. By the time a locomotive engineer sees a trespasser or vehicle on the tracks, it's too late. It takes the average freight train traveling at 55 mph more than a mile—the length of 18 football fields—to stop. Trains cannot stop quickly enough to avoid a collision.

  • Remember to cross train tracks only at designated pedestrian or roadway crossings, and obey all warning signs and signals posted there.

  • Stay alert around railroad tracks. No texting, headphones or other distractions that would prevent you from hearing an approaching train; never mix rails and recreation.

PLR 18-038 - PMV-4 Mishap Claims One Soldier's Life

A Specialist assigned to the Wisconsin Army National Guard died in a PMV-4 mishap 3 February 2018 near Westby, Wisconsin, at 1700 local. The Soldier was on his way home from his drill location when his vehicle slid across the centerline on a right-hand curve in wet road conditions and collided with an approaching vehicle.

ENGAGEMENT TIPS

  • Reduce your vehicle's speed when visibility is limited and less-than-ideal road conditions are encountered.

PLR 18-037 - Weapons Mishap Claims One Soldier's Life

A Soldier assigned to 75th Ranger Regiment, Joint Base Lewis-McChord, Washington, died in a weapons firing mishap 31 January 2018 at 1812 local at Camp Rilea, Oregon. The Soldier was struck by a discharged round during a small-arms magazine change drill. The USACRC is leading an investigation into the accident.

PLR 18-036 - PMV-4 Mishap Claims One Soldier's Life

A Captain assigned to U.S. Army Europe at Clay Kaserne died 24 January 2018 following a PMV-4 mishap in Wiesbaden, Germany, at 1805 local. The Soldier was traveling on a highway in a borrowed subcompact vehicle in light-rain conditions when he reportedly lost control and was struck by a bus traveling the same direction. The Soldier was evacuated to a hospital but passed away from his injuries.

ENGAGEMENT TIPS:

  • Conduct individual risk management. Continuously monitor conditions and make adjustments as necessary.

  • Adjust your speed for the conditions. Remember that posted speed limits are for ideal weather and road conditions.

  • Brief hazardous locations on local area roads where previous accidents have occurred.

  • When operating a borrowed PMV, ensure you are familiar with the vehicle’s handling characteristics before taking to the road.

  • When driving a mini or micro vehicle, such as a Smart car, use extra focus on the road and always drive defensively. Remember, you are operating one of the smallest vehicles on the road, which could be a disadvantage in the event of an accident.

PLR 18-035 - Parachuting Mishap Claims One Soldier's Life

A Master Sergeant assigned to U.S. Army Special Operations Command, Joint Base Myer-Henderson Hall, Virginia, died in a parachuting mishap 24 January 2018 at 1000 local in Eloy, Arizona. The Soldier's parachute reportedly failed to fully deploy and he struck the ground.
