About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-062 - PMV-4 Mishap Claims One Soldier's Life

A 19-year-old Private First Class assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a PMV-4 mishap 23 April 2018 in Clarksville, Tennessee, at 0415 local. The Soldier was operating his vehicle when it left the roadway and struck three trees. Wet road conditions were present at the time of the accident.

PLR 18-061 - Pedestrian Mishap Claims One Soldier's Life

A Corporal assigned to U.S. Army Reserve Command died in a pedestrian mishap 5 April 2018 in Orlando, Florida, at 0230 local. The Soldier was in battle assembly/drill status when he was struck by a vehicle as he crossed an intersection on foot.

PLR 18-060 - Tactical Vehicle Mishap Claims One Soldier's Life

A Private assigned to 7th Infantry Division, Joint Base Lewis-McChord, Washington, died in a tactical vehicle mishap 10 April 2018 at the National Training Center, Fort Irwin, California, at 2100 local. The Soldier was sleeping beside his assigned vehicle when a Stryker that was being positioned for parking struck him and another Solider. The second Soldier suffered non-life threatening injuries. The USACRC is leading a centralized accident investigation into the mishap.


PLR 18-059 - PMV-2 Mishap Claims One Soldier's Life

A 34-year-old Staff Sergeant assigned to Combined Arms Support Command, Joint Base Langley-Eustis, Virginia, died in a PMV-2 mishap 7 April 2018 in Newport News, Virginia, at 0029 local. The Soldier was riding his motorcycle on an interstate when he struck the rear of another vehicle and then a jersey barrier.

PLR 18-058 - Aviation Mishap Claims Two Soldiers' Lives

A 37-year-old Chief Warrant Officer 3 and 28-year-old Warrant Officer 1 assigned to 101st Aviation Division, Fort Campbell, Kentucky, died 6 April 2018 in an aviation mishap on the installation at 2140 local. At the time of the accident, the Soldiers were conducting aerial gunnery training under night vision systems. The USACRC is leading a centralized accident investigation into the mishap.
