About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-044 - Live-Fire Training Exercise Claims One Soldier's Life

A 20-year-old Specialist assigned to U.S. Army Alaska died in a live-fire mishap 30 March 2019 in Donnelly Training Area, Alaska, at 0225 local. The Soldier was struck by small-arms fire during a combined arms live-fire training exercise.

PLR 18-089 - Live-Fire Training Mishap Claims One Soldier's Life

A 19-year-old Private assigned to 101st Airborne Division (Air Assault), Fort Campbell, Kentucky, died in a training mishap 1 August 2018 on the installation at 1300 local. The Soldier was struck by a round fired from an M4A1 carbine while participating in a small-arms, live-fire training event. The USACRC is leading a safety investigation into the mishap.

PLR 18-037 - Weapons Mishap Claims One Soldier's Life

A Soldier assigned to 75th Ranger Regiment, Joint Base Lewis-McChord, Washington, died in a weapons firing mishap 31 January 2018 at 1812 local at Camp Rilea, Oregon. The Soldier was struck by a discharged round during a small-arms magazine change drill. The USACRC is leading an investigation into the accident.
