About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 20-018 - PMV-4 Mishap Claims One Soldier's Life

A 20-year-old Private Second Class assigned to Fort Drum, New York, died in a PMV-4 mishap 20 December 2019 in Jackson, Tennessee, at 1030 local. The Soldier was driving his vehicle through a construction zone on the interstate when he struck the rear of a slow-moving 18-wheeler. The Soldier was transported to a local hospital, where he was pronounced dead. The Soldier's spouse, who was a passenger in the vehicle, died at the scene. The mishap is under investigation. This is the sixth PMV-4 fatality in FY20.
