About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 20-016 - Aviation Mishap Claims Three Soldiers' Lives

A 28-year-old Chief Warrant Officer 2, 30-year-old Chief Warrant Officer 2 and 28-year-old Sergeant, all assigned to the Minnesota Army National Guard, died in an aircraft mishap 5 December 2019 near St. Cloud, Minnesota, at 1620 local. The crewmembers were conducting a maintenance test flight when they sent a mayday call shortly after takeoff. Local authorities found the aircraft at the crash site. The mishap is under investigation. This was the second UH-60 crash in FY20.



PLR 19-066 - Aviation Mishap Claims One Soldier's Life

A 33-year-old Major assigned to Fort Polk, Louisiana, died in an aviation mishap 26 September 2019 on the installation at 0034 local. The crew was conducting a MEDEVAC mission when the aircraft crashed during landing. The USACRC is leading a safety investigation into the mishap.

 

 

PLR 18-102 - Army-Operated Aircraft Mishap Claims One Soldier's Life

A Chief Warrant Officer 4 assigned to the U.S. Army Office of the Program Manager-Saudi Arabian National Guard, died in an aviation mishap 6 September 2018 in Riyadh, Saudi Arabia. The Soldier was serving as an instructor pilot in a Saudi Arabian National Guard Ministry Boeing AH-6i helicopter when it crashed while conducting a training mission. The USACRC is leading a safety investigation into the mishap.

PLR 18-098 - Civilian Aircraft Mishap Claims One Soldier’s Life

A Staff Sergeant assigned to the U.S. Army Parachute Team, Fort Bragg, North Carolina, died in a civilian aircraft mishap 25 August 2018 in Statesboro, Georgia, at 1400 local. The Soldier was participating in an off-duty skydiving event when the plane crashed following takeoff.

 

 

PLR 18-097 - Civilian Aircraft Mishap Claims One Soldier’s Life

A 34-year-old Chief Warrant Officer 3 assigned to the Massachusetts Army National Guard died in a personally owned aircraft mishap 24 August 2018 in Hanson, Massachusetts, at 1630 local. The Soldier was piloting a civilian aircraft when he crashed near an airport. At the time of the mishap, he was home on emergency leave from Title-10 mobilization in Kosovo.
