About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-054 - AMV Mishap Claims One Soldier's Life

A 30-year-old Sergeant with the Illinois Army National Guard died in an Army motor vehicle mishap 25 March 2018 in Rockford, Illinois, at 1115 local. The Soldier was a passenger in an M997 HMMWV that overturned on an interstate during a unit convoy operation from an inactive duty training location to home station. The passenger and driver were both reportedly wearing their restraints and personal protective equipment but suffered serious injuries. The passenger later died at a local hospital.

PLR 18-047 - Army Motor Vehicle Mishap Claims One Soldier's Life

A 20-year-old U.S. Army Reserve Private died 3 March 2018 in an Army motor vehicle mishap at the U.S. Army Reserve Center, Lima, Ohio, at 1300 local. The Soldier became pinned between a stationary vehicle, that he was helping move out of the maintenance bay, and a vehicle driven by another Soldier. He was in battle assembly/drill status at the time of the accident. The USACRC is leading a centralized accident investigation into the mishap.

ENGAGEMENT TIPS

Ground guides are required when wheeled and tracked vehicles are backed or when moved within an assembly area or motor pool. Additionally, ground guides are required to be properly trained according to TC 3-21.60, TC 21–305–20, and TC 21–306.

PLR 18-044 - AMV Mishap Claims One Federal Technician's Life

A federal technician assigned to the California Army National Guard died in an Army motor vehicle mishap 12 February 2018 in Barstow, California, at 1310 local. The technician was a passenger in an M1097 HMMWV that, while on a post-maintenance test drive, reportedly crossed the center line and collided with a tractor trailer.

PLR 18-033 - Army Motor Vehicle Mishap Claims One Soldier's Life

A Specialist assigned to the Florida Army National Guard died in an Army Motor Vehicle mishap 19 January 2018 in Sebring, Florida, at 1156 local. The Soldier’s unit was conducting a vehicle convoy from the Miramar Readiness Center to Avon Park Bombing Range when three M1120 LHS trucks were involved in a chain-reaction crash. Five other Soldiers were evacuated to local hospitals.

PLR 18-023 ACV Mishap Claims One Soldier's Life

A Staff Sergeant assigned to U.S. Special Operations Command, died 11 December 2017 at 1615 local in Afghanistan. The Soldier was the senior occupant in an MATV when the shoulder of the road reportedly collapsed, causing the vehicle to overturn. Seat belt use is unknown. The Soldier was deployed in support of Operation Enduring Freedom.
