About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 20-068 – PMV-2 Mishap Claims One Soldier's Life

A Sergeant assigned to Fort Carson, Colorado, died in a PMV-2 mishap 2 August 2020 in Colorado Springs, Colorado, at 0100 local. The Soldier was riding another person’s motorcycle on the highway when he lost control. Reportedly, the Soldier was speeding and crossed the median before crashing. Alcohol use is unknown at this time. The Soldier was not wearing a helmet, was not licensed, and had not completed the required Motorcycle Safety Foundation training courses. The mishap is under investigation.

Since 2016, the Army has lost an average of 28 Soldiers a year to PMV-2 mishaps. This preventable mishap is the 15th PMV-2 fatality of FY20 and below the number of fatalities for the same time period last year.

 

 

PLR 20-067 – Off-Duty Water-Related Mishap Claims One Soldier's Life

A 24-year-old Specialist assigned to Fort Hood, Texas, died in an off-duty water-related mishap 1 August 2020 at Stillhouse Hollow Lake in Bell County, Texas, at 1515 local. The Soldier was riding in an inner tube pulled by a boat when he fell into the water and did not resurface. Another Soldier in the group contacted the county sheriff's office. Divers began searching the area immediately following the arrival of law enforcement officers. The Soldier’s body was recovered by local authorities the following day and positively identified by unit leadership. The mishap is under investigation.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the eighth fatal off-duty water-related mishap of FY20 and below the number of off-duty water-related fatalities from this time last year.

 

 

PLR 20-066 – Off-Duty Water-Related Mishap Claims One Soldier's Life

A PV2 assigned to Fort Hood, Texas, died in an off-duty water-related mishap 17 July 2020 at Stillhouse Hollow Lake in Bell County, Texas, at 0630 local. On 16 July at 0630, the Soldier did not report for accountability formation. His NCOIC checked with other Soldiers and immediate family members, but they had not heard from him. Boaters discovered the Soldier’s body on the morning of the following morning. Several hours later, local county emergency service personnel confirmed that the Soldier was dead. The mishap is under investigation.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the seventh fatal off-duty water-related mishap of FY20 but still below the number of off-duty water-related fatalities from this time last year.

 

 

PLR 20-065 – GMV Mishap Claims One Soldier’s Life

A Sergeant deployed in support of Operation Inherent Resolve died in a government motor vehicle (GMV) mishap 21 July 2020 in Syria at 1716 local. The Sergeant was performing gunner duties in an M-ATV when the vehicle left the main supply route and overturned. All four crewmembers were evacuated to the local troop medical clinic, where the gunner later died. The three other Soldiers were treated for non-life-threatening injuries. The use of seat belts and the gunner restraint system is unknown at this time. The mishap is under investigation.

Since FY16, the Army has experienced an average of 19 Class A GMV mishaps resulting in the loss of 12 Soldiers per year. This is the 12th Class A GMV mishap and the 10th GMV fatality of FY20.

 

 

PLR 20-064 – GMV Mishap Claims One Soldier's Life

A Specialist assigned to Fort Drum, New York, died in a Government Motor Vehicle mishap 3 July 2020 in Farah, Afghanistan, at 1830 local. The Soldier was operating an M-RZR on a forward operating base when the vehicle overturned and pinned him under the roll bar. The CZE medics responded and began lifesaving procedures. The Soldier was transported to a medical facility, where he was pronounced dead. The mishap is under investigation.

Since 2016, the Army has lost an average of 12 Soldiers a year to on-duty GMV mishaps. This mishap is the ninth on-duty GMV fatality of FY20.
