    USACRC Fatality Alerts

    NSM 2017

    The USACRC produces one-page Fatality Alerts and sends to the field as FLASH bulletins that highlight the salient lessons from our most recent fatal Army mishaps. Using the official investigation findings, they illuminate the leading and underlying causal factors to include Soldier, leader and system failures which resulted in the loss, and provide actionable recommendations to mitigate risks to prevent the next fatality. Fatality Alerts contain information protected by DODI 6055.07 under the safety privilege and are only available to CAC holders within the .mil network.
