About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-006 - PMV-4 Mishap Claims One Soldier's Life

A Private assigned to 75th Field Artillery Brigade, Fort Sill, Oklahoma, died in a PMV-4 mishap 21 October 2017 in Apache, Oklahoma at 1000 local. The Soldier was operating a PMV-4 when it exited the roadway, struck a tree and caught fire. He was pronounced dead at the scene. Seat belt use is unknown.

PLR 18-005 - Swimming Mishap Claims One Soldier's Life

A Sergeant assigned to 1st Cavalry Division, Fort Hood, Texas, died 14 October 2017 at 2145 local in Mansfield, Texas. While reportedly swimming with friends at a local lake, the Soldier submerged and was unable to be located. He was found 30 minutes later and transported to a local medical center, where he remained in critical condition for three days before being pronounced dead.

PLR 18-004 - PMV-4 Mishap Claims One Soldier's Life

An Alaska Army National Guard Specialist died 14 October 2017 in Hope Junction, Alaska. The Soldier was traveling to her inactive-duty training station when a vehicle crossed the centerline and struck her SUV head-on. The Soldier was pronounced dead at the scene.

PLR 18-003 - Bicycling Accident Claims One Soldier's Life

A Sergeant First Class assigned to 3rd Infantry Division, Fort Stewart, Georgia, died 9 October 2017 at 0700 local on the installation. The Soldier was riding a hand-cranked trike on a public roadway near the installation gate when he was struck from behind by a Toyota Prius. He died from his injuries during transport to Winn Army Community Hospital on Fort Stewart. The Soldier was wearing a helmet and bright clothing, but his cycle had no lighting or reflective markings.

PLR 18-002 - PMV-2 Mishap Claims One Soldier's Life

A California Army National Guard Specialist died 7 October 2017 in Roseville, California. The Soldier was thrown from his motorcycle after striking the rear end of an SUV. The Soldier was in Active-Duty Special Work Status at the time of his death.
