About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-053 - POW Mishap Claims One Soldier's Life

A Soldier died in a privately owned weapon mishap 3 May 2019 in Independence, Kentucky, at 2330 local. The Soldier was handling a .38 Special handgun when he was struck by a chambered round when it accidentally discharged.
 
 

PLR 19-031 - Privately Owned Weapon Mishap Claims One Soldier's Life

A Sergeant First Class died in a privately owned weapon mishap 9 February 2019 in Colorado Springs, Colorado, at 1530 local. The Soldier was handling or cleaning his privately owned handgun at his residence when he was struck by a discharged round.

  • Treat every weapon as if it is loaded.

  • Handle every weapon with care.

  • Identify the target before you fire.

  • Never point the muzzle at anything you don’t intend to shoot.

  • Keep the weapon on safe and your finger off the trigger until you intend to fire.

PLR 19-021 - POW Mishap Claims One Soldier's Life

A 20-year-old Private First Class assigned to 101st Airborne Division (Airborne), Fort Campbell, Kentucky, died in privately owned weapon mishap 27 December 2018 in Clarksville, Tennessee, at 2045 local. The Soldier was visiting a private residence when he picked up a semi-automatic pistol that did not have a magazine inserted. While handling the weapon, a round discharged and struck the Soldier. Alcohol was reported as being involved in the mishap.

  • Firearms are designed to be deadly weapons and alcohol can have an adverse impact on your physical and mental balance. Never mix alcohol and weapons, and always THINK weapons safety!

A 25-year-old Specialist assigned to 4th Infantry Division, Fort Carson, Colorado, died in a privately owned weapon mishap 1 December 2018 in Colorado Springs, Colorado, at 2155 local. The Soldier, while hosting a party at his residence, was struck by a discharged round from a handgun that was being handled by another Soldier. Alcohol was reported as involved in the mishap.

A 26-year-old Sergeant assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a privately owned weapon mishap 12 March 2018 in Clarksville, Tennessee, at 1630 local. He was fatally injured by a discharged round while handling a privately owned weapon at his personal residence.
