About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-041 - PMV-4 Mishap Claims One Soldier’s Life

A 20-year-old Private First Class assigned to U.S. Army Alaska died in a PMV-4 mishap 15 March 2019 near Willow, Alaska, at 1130 local. The Soldier was riding in a vehicle operated by another Soldier when the driver lost control on icy road conditions and collided with a vehicle in the opposing lane.

 

 

PLR 19-040 - Parachuting Mishap Claims One Soldier’s Life

A Sergeant First Class assigned to 75th Ranger Regiment died in a military freefall parachuting mishap 15 March 2019 in Marana, Arizona, at 0500 local. The Soldier was conducting a training jump when he reportedly collided with another Soldier in midair.

 

 

PLR 19-039 - Commercial Airliner Mishap Claims One Soldier's Life

A 40-year-old Captain assigned to U.S. Army Futures Command died in a commercial airliner crash that occurred 10 March 2019 at 0844 local.

 

 

PLR 19-038 - PMV-2 Mishap Claims One Soldier's Life

A Soldier assigned to 82nd Airborne Division, Fort Bragg, North Carolina, died in a PMV-2 mishap 10 March 2019 in Fayetteville, North Carolina, at 0200 local. The Soldier was riding his motorcycle when it was struck by an approaching sedan. The civilian motorist was charged in the mishap.

PLR 19-037 - PMV-4 Mishap Claims One Soldier's Life

A Pfc. assigned to 2nd Infantry Division, Republic of Korea, died in a PMV-4 mishap 5 March 2019 in Bryan County, Georgia, at 0530 local. The Soldier was driving his pickup while on leave when he lost control and the vehicle overturned. He was reportedly not wearing a seat belt and was thrown from the vehicle.
