About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-088 - PMV-4 Mishap Claims Two Soldiers' Lives

A 28-year-old Specialist and 37-year-old Staff Sergeant assigned to the Warrior Transition Battalion, Fort Benning, Georgia, died following a PMV-4 mishap that happened 23 July 2018 in Harris County, Georgia, about 1900 local. The Soldiers’ vehicle exited the roadway, struck an embankment and overturned. Speed is suspected as a factor in the crash.

  • According to the National Highway Traffic Safety Administration, there were 37,461 traffic fatalities in 2016. Among them, 10,111 (27 percent) were in crashes where at least one driver was speeding.

  • In fatal crashes, 50 percent of speeding passenger vehicle drivers were unrestrained at the time of the crash, compared to 21 percent of non-speeding passenger vehicle drivers.

  • Young male drivers were the most likely to be speeding at the time of the fatal crash. Nearly one-third (32 percent) of male drivers in the 15- to 20-year-old age group involved in fatal crashes were speeding at the time of the crash, compared to 22 percent for female drivers in the same age group.

  • Drivers who were speeding when involved in fatal crashes were more likely to have been drinking - and drinking more - than those drivers who were not speeding.

PLR 18-087 - PMV-4 Mishap Claims One Soldier's Life

A 28-year-old Private First Class assigned to the New York Army National Guard died in a PMV-4 mishap 23 July 2018 in Yorktown, New York, at 0645 local. The Soldier was en route to his annual training duty location when his vehicle crossed the center line and struck oncoming traffic head-on.

A 27-year-old Sergeant assigned to U.S. Army Recruiting Command, Ronkonkoma, New York, died in a PMV-4 mishap 20 July 2018 in Patchogue, New York, at 0007 local. The Soldier was operating a vehicle when it reportedly exited the roadway and struck a tree. He was pronounced dead at the scene. Seat belt use is unknown.

A 31-year-old Staff Sergeant assigned to 4th Infantry Division, Fort Carson, Colorado, died in a water-related mishap 21 July 2018 at Pueblo Reservoir Lake, Pueblo, Colorado. The Soldier reportedly jumped off a rock ledge into the lake and did not resurface. A search-and-rescue team located his body the following day, at which time he was pronounced dead. The Soldier had allegedly been consuming alcohol prior to entering the water.

PLR 18-084 - Fire Mishap Claims One Army Civilian's Life

An Army Civilian Corps employee died of burns from a flash fire that occurred on 19 July 2018, at approximately 0720 local, on Letterkenny Army Depot, Pennsylvania. He and two other employees were in the process of moving a 55-gallon drum of paint thinner in the painting kitchen when the fire erupted in the area of the drum. The other two employees sustained serious injuries.
