About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-011 - PMV-4 Mishap Claims One Soldier's Life

A 21-year-old Specialist assigned to 1st Armored Division, Fort Bliss, Texas, died in a PMV-4 mishap 9 November 2018 in Allen, Texas, at 0558 local. The Soldier was traveling while on authorized pass when his vehicle left the roadway and struck a concrete barrier. A post-crash fire ensued and the Soldier was pronounced dead at the scene.

PLR 19-010 - AMV Mishap Claims One Soldier's Life

A 21-year-old Private First Class assigned to 1st Cavalry Division, Fort Hood, Texas, died in an Army motor vehicle mishap 8 November 2018 on the installation at 1615 local. The Soldier was a passenger in an M1152 HMMWV when the vehicle exited the roadway, overturned and came to rest in the upright position. He was pronounced dead at the scene.

PLR 19-009 - AMV Mishap Claims One Soldier's Life

A 21-year-old Specialist assigned to U.S. Army Special Operations Command died in an Army motor vehicle mishap 6 November 2018 on Fort Irwin, California, at 1345 local. The Soldier was a passenger in an M1151A1 HMMWV when the driver lost control on an improved gravel road. The HMMWV left the roadway and overturned. The USACRC is leading a safety investigation into the mishap.

PLR 19-008 - PMV-2 Mishap Claims One Soldier's Life

A 40-year-old Sergeant assigned to 82nd Airborne Division, Fort Bragg, North Carolina, died in a PMV-2 mishap 7 November 2018 in Bunnlevel, North Carolina, at 1800 local. The Soldier was reportedly operating a motorcycle at a high rate of speed when he exited the roadway and struck a pole. He was wearing personal protective equipment and had completed the Motorcycle Safety Foundation’s Basic RiderCourse.

ENGAGEMENT TIPS:

  • In 2016, 33 percent of all motorcycle riders involved in fatal crashes were speeding, compared
    to 19 percent for passenger car drivers, 15 percent for light truck drivers and 7 percent for large
    truck drivers.

  • Per vehicle miles traveled in 2016, motorcyclist fatalities occurred nearly 28 times more frequently
    than passenger car occupant fatalities in traffic crashes.

  • Of motorcycle riders who died in single-vehicle crashes in 2016, 37 percent were alcohol impaired.


PLR 19-007 - ACV Mishap Claims One Soldier's Life

A 23-year-old Specialist assigned to 7th Infantry Division, Joint Base Lewis-McChord, Washington, died in an Army combat vehicle mishap 4 November 2018 on the installation at 0629 local. The Soldier was resting against his assault pack when a Stryker mortar carrier backed up and pinned him to the ground. Medical personnel pronounced him dead at the scene. The USACRC is leading a safety investigation into the mishap.

ENGAGEMENT TIPS:

  • Ensure vehicle operators do a thorough walkaround the vehicle prior to moving it.

  • Proper use of a ground guide will prevent Soldiers from being run over by or pinned between vehicles.
