About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-103 - ACV Mishap Claims One Soldier's Life

A First Lieutenant assigned to 75th Ranger Regiment, Fort Benning, Georgia, died in an Army combat vehicle mishap 7 September 2018 at 2141 local. The Soldier was the tank commander of an M-1126 Stryker, supporting live-fire operations on the installation under limited visibility, when the vehicle overturned into a ditch adjacent to the road. He suffered fatal injuries, while the driver and a second Soldier sustained non-fatal injuries. The USACRC is leading a safety investigation into the mishap.

PLR 18-102 - Army-Operated Aircraft Mishap Claims One Soldier's Life

A Chief Warrant Officer 4 assigned to the U.S. Army Office of the Program Manager-Saudi Arabian National Guard, died in an aviation mishap 6 September 2018 in Riyadh, Saudi Arabia. The Soldier was serving as an instructor pilot in a Saudi Arabian National Guard Ministry Boeing AH-6i helicopter when it crashed while conducting a training mission. The USACRC is leading a safety investigation into the mishap.

PLR 18-101 - PMV-2 Mishap Claims One Soldier's Life

A 25-year-old Sergeant assigned to U.S. Army Europe, Vicenza, Italy, died in a PMV-2 mishap 4 September 2018 in Asiago, Italy, at 1600 local. The Soldier was operating a motorcycle when he lost control negotiating a curve and struck a brick wall. He was wearing a helmet. Motorcycle Safety Foundation training has not been verified.

PLR 18-100 - PMV-4 Mishap Claims One Soldier's Life

A 20-year-old Private assigned to 1st Cavalry Division, Fort Hood, Texas, died in a PMV-4 mishap 2 September 2018 in Georgetown, Texas, at 0200 local. The Soldier was a passenger in a vehicle driven by another Soldier when it struck the rear of a semi-truck/trailer. He was killed upon impact, and the driver was transported to a local hospital with injuries. Alcohol is suspected as a factor in the mishap.

ENGAGEMENT TIPS:

  • In 2016, there were 10,497 fatalities in motor vehicle traffic crashes involving drivers with a BAC of .08 percent or higher, totaling 28 percent of all traffic fatalities that year. An average of one alcohol-impaired-driving fatality occurred every 50 minutes the same year.

  • The 25- to 34-year-old age group had the highest percentage (27 percent) of drivers with a BAC of .08 percent or higher in fatal crashes compared to other age groups in 2016.

PLR 18-099 - PMV-2 Mishap Claims One Soldier’s Life

A 28-year-old Staff Sergeant assigned to 10th Mountain Division, Fort Polk, Louisiana, died in a PMV-2 mishap 24 August 2018 in Burkeville, Texas, at 1933 local. The Soldier was operating a motorcycle when he reportedly lost control after striking loose gravel on the roadway. He was thrown from the motorcycle and struck by an oncoming vehicle. The Soldier had completed the Motorcycle Safety Foundation’s Basic RiderCourse. PPE use is unknown at this time.
