About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-021 ATV Mishap Claims One Soldier's Life

A Staff Sergeant assigned to the Puerto Rico Army National Guard died 10 December 2017 at 1800 local in Orocovis, Puerto Rico. The Soldier died in an all-terrain vehicle mishap. PPE use by the Soldier is unknown.

PLR 18-020 - Training Exercise Mishap Claims One Soldier's Life

A Specialist assigned to 3rd Infantry Division, Fort Stewart, Georgia, died 12 December 2017 following a training exercise mishap on the installation. The Soldier was participating in a live-fire exercise when he was struck in the head and left bicep by fired 5.56 rounds. He underwent surgery but died from his injuries six days later.

PLR 18-019 - PMV-4 Mishap Claims One Soldier's Life

A Captain in the West Virginia Army National Guard died in a PMV-4 mishap 2 December 2017 at 1800 local in Springfield Township, Pennsylvania. The Soldier was returning to his home of record from his inactive-duty training station when his SUV was struck head-on by the driver of a service van that was traveling east in the westbound lane. The Soldier was pronounced dead at the scene.

PLR 18-018 - Pedestrian Mishap Claims One Soldier's Life

A Sergeant assigned to Eighth Army, Suwon Air Base, Republic of Korea, died in a pedestrian mishap 7 December 2017 at 2315 local near Suwon, Korea. The Soldier’s unit was conducting convoy operations when his Army Motor Vehicle was involved in an accident. In an attempt to emplace warning triangles, the Soldier exited the AMV and was struck by a civilian vehicle. He was ground evacuated to a local hospital, where he was later pronounced dead.

ENGAGEMENT TIPS:

If you are involved in a vehicle accident or other roadside emergency:

  • Pull over and out of traffic if possible. Even if all of the emergency lights are activated, some highway drivers do not pay close attention and could rear-end the disabled vehicle, causing further damage or injury.

  • If you must exit the vehicle, do so safely and well away from oncoming traffic and your vehicle.

  • If possible, you and any passengers should exit through the side of the vehicle facing away from the road.

  • Not only do you need to stay out of the roadway, you need to put as much distance as possible between you and the disabled vehicle until traffic has been diverted around it.

PLR 18-017 - Snowboarding Mishap Claims One Soldier's Life

A Sergeant assigned to 4th Infantry Division, Fort Carson, Colorado, died in a snowboarding mishap 2 December 2017 at 1245 local in Monarch, Colorado. The Soldier struck a tree while snowboarding at a ski resort. He was pronounced dead at the scene.
