About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-046 - PMV-4 Mishap Claims One Soldier's Life

A Sergeant First Class assigned to the White House Communications Agency, Joint Base Anacostia-Bolling, Washington, DC, died 25 February 2018 in a PMV-4 mishap in Fort Worth, Texas at 2020 local.

PLR 18-045 - PMV-2 Mishap Claims One Soldier's Life

A 25-year-old Specialist assigned to 101st Airborne Division, Fort Campbell, Kentucky, died 23 February 2018 in a PMV-2 mishap in Clarksville, Tennessee, at 1320 local. The Soldier was operating a sport bike, with the right of way, when he collided with a truck that turned into his path. He was licensed, Motorcycle Safety Foundation trained and wearing personal protective equipment.

ENGAGEMENT TIPS

As a rider, you can’t be sure that other vehicle operators will see you or yield the right of way. To lessen your chances of a crash occurring, be visible - ride in the best lane position to see and be seen.

Also remember to:

  • Search your path of travel 12 seconds ahead.

  • Identify and separate hazards.

  • Be prepared to act — remain alert and know how to carry out proper crash-avoidance skills.

PLR 18-044 - AMV Mishap Claims One Federal Technician's Life

A federal technician assigned to the California Army National Guard died in an Army motor vehicle mishap 12 February 2018 in Barstow, California, at 1310 local. The technician was a passenger in an M1097 HMMWV that, while on a post-maintenance test drive, reportedly crossed the center line and collided with a tractor trailer.

PLR 18-043 - PMV-2 Mishap Claims One Soldier's Life

A Sergeant First Class assigned to U.S. Army Special Operations Command, Fort Bragg, North Carolina, died 11 February 2018 in a PMV-2 mishap in Cumberland County, North Carolina, at 1636 local. The Soldier reportedly rode over a patch of gravel, lost control of his motorcycle and crashed.

A Staff Sergeant assigned to 101st Airborne Division, Fort Campbell, Kentucky, died 11 February 2018 in a PMV-2 mishap in Springfield, Tennessee, at 0845 local. The Soldier was reportedly speeding when he lost control of his motorcycle, exited the roadway and crashed. The Soldier had completed the Experienced Motorcycle Rider Course (ERC).
