About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-048 - PMV-2 Mishap Claims One Soldier's Life

A 31-year-old Warrant Officer assigned to U.S. Army Forces Command, Aberdeen Proving Ground, Maryland, died in a PMV-2 mishap 23 April 2019 in Arlington, Virginia, at 0159 local. The Soldier was operating his sport-model motorcycle at a high rate of speed on Interstate 395 North when he left the roadway, struck a concrete Jersey barrier, crossed the opposing lanes and crashed. He was thrown from the motorcycle, which caught fire during the crash sequence. The Soldier was deemed an experienced rider, having completed the Motorcycle Safety Foundation’s Experienced RiderCourse in August 2016, and was wearing personal protective equipment, but died at the scene.

ENGAGEMENT TIPS:

  • In 2016, 33 percent of all motorcycle riders involved in fatal crashes were speeding, compared to 19 percent for passenger car drivers.

  • Motorcycle riders killed in traffic crashes at night were three times more frequently alcohol impaired than those killed during the day in 2016.

PLR 19-046 - PMV-2 Mishap Claims One Soldier's Life

A 19-year-old Specialist assigned to 3rd Infantry Division died in a PMV-2 mishap 3 April 2019 on Fort Stewart, Georgia, at 1758 local. The Soldier was leaving a parking area when he lost control of his sport-model motorcycle and struck a Jersey barrier. During the mishap sequence, the Soldier’s helmet reportedly came off, resulting in a fatal head injury.

ENGAGEMENT TIPS:

Sport bikes can accelerate rapidly due to their high torque and responsiveness. Riders must have constant awareness and control when applying power from a standing start. All personal protective equipment should be worn and secured properly prior to movement.

PLR 19-045 - PMV-2 Mishap Claims One Soldier's Life

A 23-year-old Specialist assigned to U.S. Army Network Enterprise Technology Command died in a PMV-2 mishap 31 March 2019 in Oahu, Hawaii, at 1250 local. The Soldier was riding on a highway with three other Soldiers when he and another rider lost control in a series of S-curves and crashed. His motorcycle then struck a guardrail.

A 24-year-old Specialist assigned to White Sands Missile Range died in a PMV-2 mishap 23 March 2019 in Las Cruces, New Mexico, at 1748 local. The Soldier was reportedly riding his motorcycle at a high rate of speed when he collided with a vehicle at a highway exit. He was wearing personal protective equipment and had completed basic rider training.

ENGAGEMENT TIPS:

  • Identify Soldiers who exhibit high-risk behavior and counsel them.

  • Make discussions about safety and loss prevention personal.

PLR 19-038 - PMV-2 Mishap Claims One Soldier's Life

A Soldier assigned to 82nd Airborne Division, Fort Bragg, North Carolina, died in a PMV-2 mishap 10 March 2019 in Fayetteville, North Carolina, at 0200 local. The Soldier was riding his motorcycle when it was struck by an approaching sedan. The civilian motorist was charged in the mishap.
