About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-096 - PMV-2 Mishap Claims One Soldier's Life

A 21-year-old Specialist assigned to 82nd Airborne Division, Fort Bragg, North Carolina, died in a PMV-2 mishap 24 August 2018 in Fayetteville, North Carolina, at 0807 local. The Soldier was operating a motorcycle with the right of way when he collided with a truck that turned into his path of travel. He was wearing PPE and had completed the Motorcycle Safety Foundation’s Basic RiderCourse.

 

 

PLR 18-090 - PMV-2 Mishap Claims One Soldier's Life

A 24-year-old Sergeant assigned to 1st Armor Division, Fort Bliss, Texas, died in a PMV-2 mishap 5 August 2018, in El Paso, Texas, at 2015 local. The Soldier was riding on a highway with other riders when his motorcycle struck the rear end of a vehicle as it prepared to exit via an off-ramp. He was pronounced dead at the scene. Speed is suspected as a contributing factor. The Soldier had not completed the Motorcycle Safety Foundation’s Basic RiderCourse, and personal protective equipment use was not reported.

PLR 18-083 - PMV-2 Mishap Claims One Soldier's Life

A 23-year-old Second Lieutenant assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a PMV-2 mishap 16 July 2018 in Clarksville, Tennessee, at 1200 local. The Soldier was on leave while operating his motorcycle with the right of way when he collided with a vehicle that turned into his path of travel. He was wearing an approved helmet.

  • Any intersection is potentially hazardous, whether it has stoplights, stop signs or
    is unmarked.

  • The majority of multi-vehicle motorcycle crashes generally are caused when other
    drivers simply didn’t see the motorcyclist. Proceed cautiously at intersections and
    yield to pedestrians and other vehicles as appropriate.

  • Be mentally prepared for every ride and assume vehicles may pull out in front of you.

A 30-year-old Staff Sergeant assigned to U.S. Cyber Command, Fort Meade, Maryland, died in a PMV-2 mishap 20 May 2018 in Baltimore, Maryland, at 1800 local. The Soldier was operating his motorcycle on an interstate when he lost control in a curve and struck a wall.
