About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-079 - PMV-4 Mishap Claims One Soldier's Life

A 19-year-old Private assigned to 1st Cavalry Division, Fort Hood, Texas, died in a PMV-4 mishap 4 July 2018 in Gardner, Kansas, at 0130 local. The Soldier was on leave when his vehicle left the roadway in dry weather conditions and crashed. Authorities suspect speed was a factor in the crash.

 

 

PLR 18-078 - PMV-4 Mishap Claims One Soldier's Life

A 20-year-old Specialist assigned to 1st Cavalry Division, Fort Hood, Texas, died 28 June 2018 from injuries sustained in a PMV-4 mishap that occurred the prior day in Little Rock, Arkansas, at 0840 local. The Soldier was on leave when his pickup left the roadway in dry weather conditions and struck a culvert.

A 25-year-old Specialist assigned to the California Army National Guard died in a PMV-4 mishap 17 May 2018 in Carson, California, at 0343 local. The Soldier was en route to his drill location when his vehicle struck a concrete barrier.

A Specialist assigned to 101st Airborne Division, Fort Campbell, Kentucky, died 30 April 2018 from injuries sustained in PMV-4 mishap nine days earlier in Clarksville, Tennessee, at 0400 local. The Soldier was a passenger in a vehicle that went out of control on an interstate and came to a rest inverted in the median. The driver suffered minor injuries. Local authorities suspect alcohol was a factor.

  • Drivers with a blood alcohol content of .08 percent or higher involved in fatal crashes were 4.5 times more likely to have a prior conviction for DWI than drivers with no alcohol in their systems.

  • Every day, nearly 29 people in the U.S. die in alcohol-impaired vehicle crashes; that's one person every 50 minutes.

  • The 25- to 34-year-old age group had the highest percentage (27 percent) of drivers with a BAC of .08 percent or higher in fatal crashes compared to other age groups.

A 26-year-old Sergeant assigned to 1st Battalion, 507th Parachute Infantry Regiment, Fort Benning, Georgia, died in a PMV-4 mishap 29 April 2018 in Tallahassee, Florida, at 0044 local. The Soldier was operating a vehicle when it reportedly left the roadway, struck numerous trees and overturned. He was ejected from the vehicle and pronounced dead at the scene. According to law enforcement officials, he was not wearing a seat belt.

  • Forty-eight percent of passenger vehicle occupants who were killed in traffic crashes in 2016 were unrestrained.

  • Seat belts saved an estimated 14,668 lives of passenger vehicle occupants 5 and older in 2016.

  • Research has found that lap/shoulder seat belts, when used, reduce the risk of fatal injury to front-seat passenger car occupants by 45 percent and the risk of moderate-to-critical injury by 50 percent.
