About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-015 - PMV-4 Mishap Claims One Soldier's Life

A Specialist assigned to the 10th Mountain Division, Fort Polk, Louisiana, died in a PMV-4 mishap 30 November 2017 at 2330 local in Leesville, Louisiana. The Soldier was a passenger in a vehicle, driven by another Soldier, that was reportedly traveling at a high rate of speed. The vehicle left the road and struck a tree, causing the Soldier to suffer fatal injuries. The driver incurred non-life-threating injuries. Both Soldiers were wearing seat belts.

PLR 18-013 - PMV-4 Mishap Claims One Soldier's Life

A Private First Class assigned to 16th Military Police Brigade, Fort Stewart, Georgia, died in a PMV-4 mishap 23 November 2017 at 0937 local in Ocala, Florida. The Soldier reportedly lost control of her vehicle while negotiating a curve, crossed the center line and collided with a pickup truck. She was pronounced dead at the scene. The Soldier was wearing a seat belt.

PLR 18-012 - PMV-4 Mishap Claims One Soldier's Life

A First Lieutenant assigned to 1st Infantry Division, Fort Riley, Kansas, died in a PMV-4 mishap 18 November 2017 at 0030 local near Fort Stockton, Texas. According to reports, the Soldier was traveling west on I-10 when her vehicle was struck by an eastbound vehicle that crossed the center median. She was pronounced dead at the scene. The Soldier was in PCS status at the time of the accident. Seat belt use is unknown.

PLR 18-006 - PMV-4 Mishap Claims One Soldier's Life

A Private assigned to 75th Field Artillery Brigade, Fort Sill, Oklahoma, died in a PMV-4 mishap 21 October 2017 in Apache, Oklahoma at 1000 local. The Soldier was operating a PMV-4 when it exited the roadway, struck a tree and caught fire. He was pronounced dead at the scene. Seat belt use is unknown.

PLR 18-004 - PMV-4 Mishap Claims One Soldier's Life

An Alaska Army National Guard Specialist died 14 October 2017 in Hope Junction, Alaska. The Soldier was traveling to her inactive-duty training station when a vehicle crossed the centerline and struck her SUV head-on. The Soldier was pronounced dead at the scene.
