About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-056 - PMV-4 Mishap Claims One Soldier's Life

A 20-year-old Private assigned to 10th Mountain Division, Fort Drum, New York, died in a PMV-4 mishap 30 March 2018 in Pittsburgh, Pennsylvania, at 0309 local. The Soldier’s vehicle was struck head on by a vehicle that was traveling in the wrong direction on the interstate.

PLR 18-055 - Pedestrian Mishap Claims One Soldier's Life

A 31-year-old Captain assigned to 4th Infantry Division, Fort Carson, Colorado, died in a pedestrian mishap 24 March 2018 in Colorado Springs at 0221 local. The Soldier was suspected to have been sitting on or near railroad tracks when he was struck by a passing train.

ENGAGEMENT TIPS:

  • All train tracks are private property. Never walk or sit on tracks; it's illegal trespass and highly dangerous. By the time a locomotive engineer sees a trespasser or vehicle on the tracks, it's too late. It takes the average freight train traveling at 55 mph more than a mile—the length of 18 football fields—to stop. Trains cannot stop quickly enough to avoid a collision.

  • Remember to cross train tracks only at designated pedestrian or roadway crossings, and obey all warning signs and signals posted there.

  • Stay alert around railroad tracks. No texting, headphones or other distractions that would prevent you from hearing an approaching train; never mix rails and recreation.


PLR 18-054 - AMV Mishap Claims One Soldier's Life

A 30-year-old Sergeant with the Illinois Army National Guard died in an Army motor vehicle mishap 25 March 2018 in Rockford, Illinois, at 1115 local. The Soldier was a passenger in an M997 HMMWV that overturned on an interstate during a unit convoy operation from an inactive duty training location to home station. The passenger and driver were both reportedly wearing their restraints and personal protective equipment but suffered serious injuries. The passenger later died at a local hospital.

PLR 18-053 - Kayaking Mishap Claims Two Soldiers' Lives

A 22-year-old Private First Class and 21-year-old Private, both assigned to 7th Infantry Division, Joint Base Lewis-McChord, Washington, died following a kayaking mishap at an installation lake 19 March 2018 at 2352 local. The Soldiers are believed to have died from complications caused by hypothermia. Both were reportedly wearing personal flotation devices.

PLR 18-052 - PMV-4 Mishap Claims One Soldier's Life

A 19-year-old Private assigned to 20th Engineering Brigade, Fort Bragg, North Carolina, died in a PMV-4 mishap 17 March 2018 in Floyd County, Indiana, at 0230 local. The Soldier’s vehicle exited the interstate, traveled into the median and rolled numerous times before coming to a rest inverted. He was reportedly not wearing a seat belt.

ENGAGEMENT TIPS:

  • Forty-eight percent of passenger vehicle occupants who were killed in traffic crashes in 2016 were unrestrained.

  • Seat belts saved an estimated 14,668 lives of passenger vehicle occupants 5 and older in 2016.

  • Research has found that lap/shoulder seat belts, when used, reduce the risk of fatal injury to front-seat passenger car occupants by 45 percent and the risk of moderate-to-critical injury by 50 percent.
