About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 19-028 - Boating Mishap Claims One Soldier's Life

A Sergeant assigned to 69th Air Defense Artillery Brigade, Fort Hood, Texas, died in a boating mishap 21 January 2019 on Belton Lake at 1500 local. The Soldier was fishing with another individual when their boat capsized and he failed to resurface. His body was later recovered.

PLR 19-004 - Swimming Mishap Claims One Soldier's Life

A Private First Class assigned to Joint Task Force-Guantanamo Bay, Cuba, died following a swimming mishap 16 October 2018 at Cable Beach, Guantanamo Bay, at 1300 local. The Soldier reportedly was overcome by the current while swimming in coastal waters with other Soldiers. He was in Title-10/activated status.

A 28-year-old Specialist assigned to U.S. Army Europe died in a swimming mishap 6 August 2018 at Riedsee Bei Leeheim, Hesse, Germany, at 1650 local. The Soldier swam with a group to a large inflatable approximately 30-40 meters out, and submerged while returning to shore. He was recovered and transported to a local hospital, where he died from water inhalation complications.

A 31-year-old Staff Sergeant assigned to 4th Infantry Division, Fort Carson, Colorado, died in a water-related mishap 21 July 2018 at Pueblo Reservoir Lake, Pueblo, Colorado. The Soldier reportedly jumped off a rock ledge into the lake and did not resurface. A search-and-rescue team located his body the following day, at which time he was pronounced dead. The Soldier had allegedly been consuming alcohol prior to entering the water.

PLR 18-073 - ATV Mishap Claims One Soldier's Life

A 19-year-old Private assigned to 4th Infantry Division, Fort Carson, Colorado, died in an ATV mishap 2 June 2018 in Gallitzin, Pennsylvania. The Soldier, who was on leave, was found dead after having been thrown from an ATV. He was not wearing a helmet at the time of the crash. Local authorities are focusing on speed and possibly alcohol as factors in the accident.
