About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 20-062 – Off-Duty Water-Related Mishap Claims One Soldier's Life

A Private First Class assigned to Fort Campbell, Kentucky, died in an off-duty water-related mishap 28 June 2020 in Dover, Tennessee, at 1245 local. A group of Soldiers were swimming when one of them began struggling after entering the water from a rope swing. Another Soldier attempted to help, but the Soldier never resurfaced. Within 10 minutes, the other Soldiers alerted local authorities, who organized a search-and-rescue mission with three sonar-equipped boats in the vicinity of where the missing Soldier was last seen. The Soldier’s body was recovered the following morning.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the sixth fatal off-duty water-related mishap of FY20 and below the number of off-duty water-related fatalities from this time last year.

 

 

PLR 20-058 – Off-Duty Water-Related Mishap Claims One Soldier's Life

A Sergeant First Class assigned to Fort Bragg, North Carolina, died in an off-duty water-related mishap 20 June 2020 in Myrtle Beach, South Carolina, at 1200 local. The Soldier was swimming off shore with his child when they were caught in a rip current. The child was rescued and returned safely to the Soldier's spouse. Lifeguards performed CPR on the Soldier but were unable to resuscitate him. Alcohol use is unknown at this time.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the fifth fatal off-duty water-related mishap of FY20 and below the number of off-duty water-related fatalities from this time last year.

 

 


PLR 20-056 – Off-Duty Water-Related Mishap Claims One Soldier's Life

A Private First Class assigned to Fort Carson, Colorado, died in an off-duty water-related mishap 12 June 2020 in Pueblo, Colorado, at 1330 local. The Soldier submerged underwater while swimming with other Solders at a local state park reservoir. Local law enforcement agencies responded to the scene with sonar and an underwater drone. The Soldier’s body was recovered several hours later and he was pronounced dead by the coroner. It is suspected that an abrupt drop-off into deeper water may have contributed to the drowning. Criminal Investigation Division and unit personnel were notified of the incident, and mishap is under investigation.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the fourth fatal off-duty water-related mishap of FY20 and below the number of off-duty water-related fatalities from this time last year. As the Army emerges from COVID-19, Soldiers must remember that risk management is critical on and off duty.

 

 

PLR 20-055 – Off-Duty Water-Related Mishap Claims One Soldier's Life

A Specialist assigned to Fort Carson, Colorado, died in an off-duty water-related mishap 12 June 2020 in Pueblo, Colorado, at 1200 local. A group of Soldiers were swimming in a local state park reservoir when one of them reportedly submerged underwater and did not resurface. His friends immediately notified emergency management services, park responders and their chain of command. The park service employed divers and sonar to locate the Soldier’s body. He was positively identified by the battalion commander. Alcohol use is unknown at this time.

Since FY16, the Army has lost an average of eight Soldiers a year to off-duty water-related mishaps. This tragedy was the third fatal off-duty water-related mishap of FY20 and below the number of off-duty water-related fatalities from this time last year. As the Army emerges from COVID-19, Soldiers must remember that risk management is critical on and off duty.

 

 

PLR 20-045 - Sports-Related Mishap Claims One Soldier's Life

A 45-year-old Staff Sergeant assigned to Fort Gordon, Georgia, died in an off-duty bicycling mishap 29 April 2020 in Evans, Georgia, at 0530 local. The Soldier was riding on an approved trail when the mishap occurred. He was found lying on the ground by a motorist and transported to the local hospital, where he was evaluated and admitted to the intensive care unit with a skull fracture and bruising to the brain. The Soldier was medically sedated and monitored by the attending neurosurgeon. Five days later, the care team attempted to bring him out of sedation, but he did not respond well. The following day, the Soldier underwent surgery, but his condition deteriorated. He died from his injuries two days later. The Soldier was wearing a reflective shirt and vest, but no helmet.

Since FY16, the Army has lost an average of five Soldiers a year to off-duty sports-related mishaps. This was the first off-duty sports-related mishap of FY20 and below the number of similar fatalities from this time last year.
