About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-069 - Boating Mishap Claims One Soldier’s Life

A 20-year-old Private First Class assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a boating mishap 11 May 2018 in Golden Pond, Kentucky, at 1500 local. The Soldier was on a raft tethered to a pontoon boat when he submerged underwater. Other Soldiers attempted to save him, but were unsuccessful. The Soldier’s body was located three days later by search and rescue personnel. He reportedly had not been wearing a personal flotation device.

PLR 18-053 - Kayaking Mishap Claims Two Soldiers' Lives

A 22-year-old Private First Class and 21-year-old Private, both assigned to 7th Infantry Division, Joint Base Lewis-McChord, Washington, died following a kayaking mishap at an installation lake 19 March 2018 at 2352 local. The Soldiers are believed to have died from complications caused by hypothermia. Both were reportedly wearing personal flotation devices.

PLR 18-021 ATV Mishap Claims One Soldier's Life

A Staff Sergeant assigned to the Puerto Rico Army National Guard died 10 December 2017 at 1800 local in Orocovis, Puerto Rico. The Soldier died in an all-terrain vehicle mishap. PPE use by the Soldier is unknown.

PLR 18-017 - Snowboarding Mishap Claims One Soldier's Life

A Sergeant assigned to 4th Infantry Division, Fort Carson, Colorado, died in a snowboarding mishap 2 December 2017 at 1245 local in Monarch, Colorado. The Soldier struck a tree while snowboarding at a ski resort. He was pronounced dead at the scene.

PLR 18-010 - Skateboarding Mishap Claims One Soldier's Life

A Captain assigned to Defense POW/MIA Accounting Agency, Joint Base Pearl Harbor-Hickam, Hawaii, died in a skateboarding mishap 30 October 2017 at 0037 local while TDY in Daegu, Republic of Korea. The Soldier was reportedly crossing an intersection on an electric skateboard when he ran into a traffic cone, fell onto the street and was struck by a passing vehicle. The Soldier was transported to a local hospital, where he was pronounced dead.
