About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-071 - PMV-2 Mishap Claims One Soldier's Life

A Staff Sergeant assigned to U.S. Army Special Operations Command, Joint Base Lewis-McChord, Washington, died in a PMV-2 mishap 19 May 2018 in Roy, Washington, at 1250 local. The Soldier was reportedly operating a motorcycle on a rural road when, after cresting a hill, he encountered two vehicles stopped in the roadway. The motorcycle rear ended one of the vehicles, resulting in the Soldier’s fatal injuries. Speed was reported as a factor in the accident.

A 25-year-old Specialist assigned to the California Army National Guard died in a PMV-4 mishap 17 May 2018 in Carson, California, at 0343 local. The Soldier was en route to his drill location when his vehicle struck a concrete barrier.

PLR 18-069 - Boating Mishap Claims One Soldier’s Life

A 20-year-old Private First Class assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a boating mishap 11 May 2018 in Golden Pond, Kentucky, at 1500 local. The Soldier was on a raft tethered to a pontoon boat when he submerged underwater. Other Soldiers attempted to save him, but were unsuccessful. The Soldier’s body was located three days later by search and rescue personnel. He reportedly had not been wearing a personal flotation device.

A 25-year-old Sergeant assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in PMV-2 mishap 8 May 2018 in Clarksville, Tennessee, at 2058 local. The Soldier was operating his motorcycle with the right of way when he collided with an SUV that turned left in his path. The Soldier was wearing his personal protective equipment but died at the scene.

PLR 18-067 - Army Motor Vehicle Mishap Claims One Soldier's Life

A Private assigned to the Tennessee Army National Guard died in an Army motor vehicle mishap 7 May 2018 at the National Training Center, Fort Irwin, California, at 0530 local. The Soldier was attempting to connect a HEMTT and a fuel pod trailer when she was pinned between them. The USACRC is leading a centralized accident investigation into the incident.

  • Use ground guides when operating in congested areas and while backing.
  • Remind operators to immediately stop the vehicle if they lose sight of the ground guide.
  • Remind Soldiers to never place themselves between a vehicle that is in operation and a stationary object unless it is properly chocked.
