About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-049 - POW Mishap Claims One Soldier's Life

A 26-year-old Sergeant assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a privately owned weapon mishap 12 March 2018 in Clarksville, Tennessee, at 1630 local. He was fatally injured by a discharged round while handling a privately owned weapon at his personal residence.

PLR 18-048 - PMV-4 Mishap Claims One Soldier's Life

A 25-year-old Private assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a PMV-4 mishap 10 March 2018 in Mayfield, Kentucky, at 0200 local. The Soldier was operating a PMV-4 when it exited the roadway, traveled sideways through a field and struck a tree. Excessive speed and wet road conditions are reported as contributing factors.

ENGAGEMENT TIPS:

  • Slow down at the first sign of rain, drizzle, or snow on the road as this is when many road surfaces are most slippery because moisture mixes with oil and dust that has not been washed away.

  • Be sure to turn on your low-beam headlights in reduced visibility instances.

  • Heavy rainfall can reduce visibility to zero. Pull over and wait for the rain to subside, or until visibility is restored.

PLR 18-047 - Army Motor Vehicle Mishap Claims One Soldier's Life

A 20-year-old U.S. Army Reserve Private died 3 March 2018 in an Army motor vehicle mishap at the U.S. Army Reserve Center, Lima, Ohio, at 1300 local. The Soldier became pinned between a stationary vehicle, that he was helping move out of the maintenance bay, and a vehicle driven by another Soldier. He was in battle assembly/drill status at the time of the accident. The USACRC is leading a centralized accident investigation into the mishap.

ENGAGEMENT TIPS

Ground guides are required when wheeled and tracked vehicles are backed or when moved within an assembly area or motor pool. Additionally, ground guides are required to be properly trained according to TC 3-21.60, TC 21–305–20, and TC 21–306.

A Sergeant First Class assigned to the White House Communications Agency, Joint Base Anacostia-Bolling, Washington, DC, died 25 February 2018 in a PMV-4 mishap in Fort Worth, Texas at 2020 local.

PLR 18-045 - PMV-2 Mishap Claims One Soldier's Life

A 25-year-old Specialist assigned to 101st Airborne Division, Fort Campbell, Kentucky, died 23 February 2018 in a PMV-2 mishap in Clarksville, Tennessee, at 1320 local. The Soldier was operating a sport bike, with the right of way, when he collided with a truck that turned into his path. He was licensed, Motorcycle Safety Foundation trained and wearing personal protective equipment.

ENGAGEMENT TIPS

As a rider, you can’t be sure that other vehicle operators will see you or yield the right of way. To lessen your chances of a crash occurring, be visible - ride in the best lane position to see and be seen.

Also remember to:

  • Search your path of travel 12 seconds ahead.

  • Identify and separate hazards.

  • Be prepared to act — remain alert and know how to carry out proper crash-avoidance skills.
