About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-053 - Kayaking Mishap Claims Two Soldiers' Lives

A 22-year-old Private First Class and 21-year-old Private, both assigned to 7th Infantry Division, Joint Base Lewis-McChord, Washington, died following a kayaking mishap at an installation lake 19 March 2018 at 2352 local. The Soldiers are believed to have died from complications caused by hypothermia. Both were reportedly wearing personal flotation devices.

PLR 18-052 - PMV-4 Mishap Claims One Soldier's Life

A 19-year-old Private assigned to 20th Engineering Brigade, Fort Bragg, North Carolina, died in a PMV-4 mishap 17 March 2018 in Floyd County, Indiana, at 0230 local. The Soldier’s vehicle exited the interstate, traveled into the median and rolled numerous times before coming to a rest inverted. He was reportedly not wearing a seat belt.

ENGAGEMENT TIPS:

  • Forty-eight percent of passenger vehicle occupants who were killed in traffic crashes in 2016 were unrestrained.

  • Seat belts saved an estimated 14,668 lives of passenger vehicle occupants 5 and older in 2016.

  • Research has found that lap/shoulder seat belts, when used, reduce the risk of fatal injury to front-seat passenger car occupants by 45 percent and the risk of moderate-to-critical injury by 50 percent.

PLR 18-051 - PMV-2 Mishap Claims One Soldier's Life

A 21-year-old Private First Class assigned to 3rd Infantry Division, Fort Stewart, Georgia, died in a PMV-2 mishap 15 March 2018 in Pembroke, Georgia, at 1850 local. The Soldier was operating his motorcycle when it struck a minivan. He was wearing his personal protective equipment and had completed Motorcycle Safety Foundation training.

A 46-year-old Major assigned to Joint Force Headquarters Montana, but serving on Active Guard Reserve orders at the Professional Education Center, Camp Joseph T. Robinson, Arkansas, died in a PMV-2 mishap 14 March 2018 in Jacksonville, Arkansas, at 0730 local. The Soldier was traveling through a construction zone, en route to his duty location, when a tractor-trailer struck his motorcycle. He was wearing his personal protective equipment and had completed Motorcycle Safety Foundation training.

PLR 18-049 - POW Mishap Claims One Soldier's Life

A 26-year-old Sergeant assigned to 101st Airborne Division, Fort Campbell, Kentucky, died in a privately owned weapon mishap 12 March 2018 in Clarksville, Tennessee, at 1630 local. He was fatally injured by a discharged round while handling a privately owned weapon at his personal residence.
