About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-034 - Aviation Mishap Claims Two Soldiers' Lives

A First Lieutenant and a Chief Warrant Officer 2 assigned to 4th Infantry Division, Fort Carson, Colorado, died in an AH-64E mishap 20 January 2018 at the National Training Center, Fort Irwin, California, at 0017 local. The crew was conducting a training flight under night vision systems in support of rotational training when their aircraft crashed.

PLR 18-033 - Army Motor Vehicle Mishap Claims One Soldier's Life

A Specialist assigned to the Florida Army National Guard died in an Army Motor Vehicle mishap 19 January 2018 in Sebring, Florida, at 1156 local. The Soldier’s unit was conducting a vehicle convoy from the Miramar Readiness Center to Avon Park Bombing Range when three M1120 LHS trucks were involved in a chain-reaction crash. Five other Soldiers were evacuated to local hospitals.

PLR 18-032 - PMV-4 Mishap Claims One Soldier's Life

A Private First Class assigned to 75th Field Artillery Brigade, Fort Sill, Oklahoma, died 5 January 2018, at 1520 local, on Interstate Highway 44, in Chickasha, Oklahoma. After another vehicle on the highway struck his 1993 Chevrolet Camaro, a fire ensued and the Soldier perished inside the vehicle.

PLR 18-031 - POW Mishap Claims One Soldier's Life

A Sergeant First Class assigned to U.S. Army Recruiting Command died 7 January 2018 from a hunting-related mishap that occurred four days prior on 4 January, in Marquez, Texas. A family member accidently shot the Soldier, who was on leave, while handling a personally owned weapon.

PLR 18-030 - PMV-4 Mishap Claims One Soldier's Life

A Staff Sergeant assigned to 1st Engineer Brigade, Fort Leonard Wood, Missouri, died 2 January 2018 at 0827 local in Hays, Kansas. The Soldier was operating a PMV-4 when it crossed over an interstate median and collided with an oncoming vehicle. She was pronounced dead at the scene. Seat belt use is unknown.
