About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-023 ACV Mishap Claims One Soldier's Life

A Staff Sergeant assigned to U.S. Special Operations Command, died 11 December 2017 at 1615 local in Afghanistan. The Soldier was the senior occupant in an MATV when the shoulder of the road reportedly collapsed, causing the vehicle to overturn. Seat belt use is unknown. The Soldier was deployed in support of Operation Enduring Freedom.

PLR 18-022 Fire Mishap Claims One Soldier's Life

A Specialist assigned to 16th Sustainment Brigade, Baumholder, Germany, died 11 December 2017 at 2130 local in Nohen, Germany. A fire reportedly started at the Soldier’s off-post residence where he was found unresponsive by firefighters and later pronounced dead.

PLR 18-021 ATV Mishap Claims One Soldier's Life

A Staff Sergeant assigned to the Puerto Rico Army National Guard died 10 December 2017 at 1800 local in Orocovis, Puerto Rico. The Soldier died in an all-terrain vehicle mishap. PPE use by the Soldier is unknown.

PLR 18-020 - Training Exercise Mishap Claims One Soldier's Life

A Specialist assigned to 3rd Infantry Division, Fort Stewart, Georgia, died 12 December 2017 following a training exercise mishap on the installation. The Soldier was participating in a live-fire exercise when he was struck in the head and left bicep by fired 5.56 rounds. He underwent surgery but died from his injuries six days later.

PLR 18-019 - PMV-4 Mishap Claims One Soldier's Life

A Captain in the West Virginia Army National Guard died in a PMV-4 mishap 2 December 2017 at 1800 local in Springfield Township, Pennsylvania. The Soldier was returning to his home of record from his inactive-duty training station when his SUV was struck head-on by the driver of a service van that was traveling east in the westbound lane. The Soldier was pronounced dead at the scene.
