About Preliminary Loss Reports (PLRs)

PLRs are intended to be used as an engagement tool for leaders to discuss the hazards and trends impacting Soldier safety and readiness. A PLR contains only basic information, as the investigation is ongoing, but provides sufficient background to allow leaders an opportunity to communicate risk at the Soldier level.

 

PLR 18-107 - PMV-4 Mishap Claims One Soldier's Life

A 25-year-old First Lieutenant assigned to U.S. Army Reserve Command died in a PMV-4 mishap 15 September 2018 in Laredo, Texas, at 1700 local.

PLR 18-106 - PMV-2 Mishap Claims One Soldier's Life

A 20-year-old Private First Class assigned to 1st Infantry Division, Fort Riley, Kansas, died in a PMV-2 mishap 15 September 2018 at 2330 local. The Soldier was riding his motorcycle when he crashed at a traffic circle on the installation. He had sport bike rider training and was wearing his personal protective equipment at the time of the mishap.

PLR 18-105 - Volunteer Firefighting Mishap Claims One Soldier's Life

A 43-year-old Staff Sergeant assigned to the South Dakota Army National Guard died in a volunteer firefighting mishap 7 September 2018 in Tilford, South Dakota, at 1640 local. The Soldier was serving as a volunteer firefighter and assisting with a structural fire when a propane tank exploded, killing him. The Soldier was on approved leave.

A 50-year-old Sergeant First Class assigned to the Alabama Army National Guard died in a PMV-2 mishap 6 September 2018 near Pass Christian, Mississippi, at 0608 local. The Soldier was operating a motorcycle on the interstate while on leave when it was reportedly struck in the rear by a vehicle. He recovered from the initial impact and was attempting to remove his motorcycle from the roadway when he was struck by a pickup truck. The Soldier had completed the Basic RiderCourse, but personal protective equipment use has not been verified.

PLR 18-103 - ACV Mishap Claims One Soldier's Life

A First Lieutenant assigned to 75th Ranger Regiment, Fort Benning, Georgia, died in an Army combat vehicle mishap 7 September 2018 at 2141 local. The Soldier was the tank commander of an M-1126 Stryker, supporting live-fire operations on the installation under limited visibility, when the vehicle overturned into a ditch adjacent to the road. He suffered fatal injuries, while the driver and a second Soldier sustained non-fatal injuries. The USACRC is leading a safety investigation into the mishap.
